Per Iowa Senate File 496 school districts are required to have parental consent for any survey of student mental, emotional, or physical health that is not required by state or federal law. In order for the district to obtain consent, parents and/or guardians will need to go to their Infinite Campus portal and provide permission. Please view this letter for additional details. There is a description of each survey and a copy of the survey is linked into the permission form if you would like to see additional details. The surveys that are included within the descriptions are surveys students have been completing in previous school years. All buildings will provide additional communication to families closer to the date any survey will be administered to your student(s). You may change your survey consent at any time by contacting your child’s attendance center or the Central Office.

This data provides important information to guide how we support all students with their social, emotional learning as well as providing information to families about other community resources.
