We are aware of recent concerns regarding student flu vaccinations administered at Cardinal Elementary. The Maquoketa Community School District, in partnership with the Department of Public Health, takes parent permissions and student health very seriously.
Following the concerns raised, we conducted a thorough review of the flu shot consent forms in collaboration with public health staff. In each case so far, we have verified that a signed parental consent form was on file prior to the vaccination being administered. It’s possible some families may not recall signing the forms due to the time between submission and the clinic date.
To ensure transparency, we are sending home copies of signed consent forms for any student who received the flu shot during our recent clinic. If you have any questions or are unsure whether you signed a form, please contact your child’s elementary building.
We appreciate the community’s partnership in keeping our students safe and healthy, and we remain committed to following all laws and procedures related to parental consent.
